These two authors discussed how they would approach counseling an epilepsy patient regarding the genetics and inheritance of epilepsy. How does one undertake a discussion for which there are so few clear answers, and when the answer is clear, the consequences are so unwelcome? This seems to be the nature of genetic disorders; knowledge of their presence in a family opens a Pandora’s Box of risk management, including the illegal discrimination by insurance companies, as stated by Author #1. Both authors showed amazing skill in going from the general to the particular that is from the general risk of epilepsy when there is no evidence of inheritance, to the risk for this patient, whom both authors thought had the possibility of carrying the diagnosis of Autosomal Dominant Frontal Lobe Epilepsy. Both authors showed appropriate sensitivity to the implications that genetic exploration would have for the patient and her family, and both sought partnership with medical genetics in guiding the care and counseling of this patient.
